Crouch Clinic Weight Loss

Weight Loss New Patient visits

This includes a visit ($145) with the provider, complete blood work ($130), Nutritional counseling, diet and exercise app, weight loss treatment plan and Vitamin B12 injection. It includes 30 minutes consultation with our Certified Holistic Nutritionist Tressa Knapp.

Returning Patients- follow up visits in person

Patients returning visits who have been seen in the office or by phone within past 3 months for weight loss only.

*(If thyroid management or complex visits with Peptides or LDN plus weight loss with appetite suppressants, the visit is $85)-must come every 3 months for thyroid Rx refills.

Returning Patients - Follow up visits in person >3 months

Phone Consultation

This visit is for patients who have been seen in the past 3 months in the office and qualify for a phone consultation with the Provider for weight loss management.

*(If thyroid management in addition to weight loss, the visit is $85, provider will give 3 months RX at a time and patient needs to be seen every 3 months in person.)

Thyroid only visits

$195

This visit is for patients who we are managing their Thyroid condition or who used to be in our weight loss program and have not been seen in the past 3 months for weight loss and we are managing their thyroid. Visits may need to be 1-2 x yearly or more often if closer monitoring is needed.

Labs are required yearly ( Superchem or Superchem Plus) and for thyroid management after RX adjustment, the cost is $50-85.

Additional Services

Vitamin B12 injection:

$20

Methylcobalamin is considered the most potent form of Vitamin B12 found in nature and the only active form of Vitamin B-12 in the brain outside the mitochondria. We need methylcobalamin for the healthy development and sustenance of our circulatory, immune and nervous systems. B12 is needed for energy and may assist with weight loss.

If not given with a visit, costs $25.

Lipo Lean Injection

Combination of B12, Methionine, choline and Inositol - may help with weight loss, fat burning, detox, focusing, etc.

Superchem

This is a complete blood work panel done through the Hamilton Lab, includes Complete blood count (CBC), Liver Enzymes, Lipids, Kidney function, Serum Magnesium, Thyroid function ( TSH, Free T3, Free T4), Insulin and Hgb A1C.

Superchem Plus

$180

This is a complete blood work panel done through the Hamilton Lab, includes Vitamin D25 OH, Complete blood count (CBC), Liver Enzymes, Lipids, Kidney function, Serum Magnesium, Thyroid function (TSH, Free T3, Free T4), Insulin and Hgb A1C.
