About Our Clinic

History of the Crouch Clinic


The Crouch Clinic has been in the same location in Hamilton, Texas for more than 30 years and primarily treats patients for weight control and diseases associated with over-weight and obesity.

The Crouch Clinic Weight Loss Program utilizes weight control principles developed over the more than 40 years of clinical experience, medical education and research by N. C. Crouch, D.O. (deceased) and Mark Crouch, D.O. These principles include but are not limited to: dietary changes for weight loss while maintaining health, exercise specific for fat burning, techniques to positively influence neurotransmitters and hormones involved in fat burning and appetite control, motivational techniques, weight maintenance techniques, proprietary formulations of compounded prescription medication and proprietary formulations of vitamin and mineral combinations. These principles are updated as new research and information becomes available.

The Crouch Clinic Weight Control Program utilizes approved appetite suppressants including time release formulations of phentermine that are not available commercially and are compounded to the specific formulations developed by Mark Crouch, D.O. and compounding pharmacist Danny Ray, RPh.

The Crouch Clinic Weight Control Program utilizes specific vitamin and mineral formulations for weight control. One such formulation was developed by Mark Crouch, D.O. and is manufactured exclusively for the patients of the Crouch Clinic. These formulations are composed strictly of vitamins and minerals with well established Dietary Reference Intake recommendations and do not contain herbal compounds. These vitamin and mineral formulations are within widely accepted therapeutic doses in combinations that are not available commercially.

The Crouch Clinic utilizes the Crouch Clinic Patient Education System which is a unique library of articles written and/or compiled by the Drs. Crouch over the past 40 years. This library is updated regularly and is designed to motivate and inform patients about well established weight loss principles as well as new information from the latest studies.

Services Provided

The Crouch Clinic is a Weight Control clinic. We provide medically supervised weight loss services for the treatment of obesity, overweight and associated medical conditions. We do not do bariatric surgery. 



Payment in full is expected at the time of service. We accept Cash, Personal Check (with proper identification), Major Credit and Debit Cards, Money Orders, and Cashier's Checks. We do not accept two party or temporary checks. 


We do not accept or file insurance. At your request, we will provide you with the information you need to file your own insurance for reimbursement directly to you. Most insurance policies do not provide coverage for weight control services or medications. 


The Crouch Clinic offers treatment for weight control and certain medical conditions associated with overweight and obesity. You should continue to see your primary care provider for routine health maintenance and screening and management for all other medical problems. The Crouch Clinic will refer you back to your primary care provider for medical problems that are beyond the scope of the practice that may be discovered by lab work or on physical examination.  

Dr. Crouch does not provide any medical services via this website or the internet. All medical services are provided by direct patient contact, evaluation and interaction at the Crouch Clinic located at 113 East Leslie Hamilton, Texas 76531.

Individual weight loss results will vary from person to person. Success is dependent on many factors many of which are out of the control of Dr. Crouch and the Crouch Clinic. Dr Crouch offers no guarantee or warranty as to the rate of weight loss or success. For certain patients with severe weight problems or complex medical conditions, preventing further weight gain or slowing the rate of weight gain may have a significantly positive impact on health.  

The Crouch Clinic protects the privacy and confidentiality of its patients. None of the patients of the Crouch Clinic appear on this website. All images and photos appearing on this website are of models or actors portraying the healthy diet and lifestyle promoted by Dr. Crouch and the staff of the Crouch Clinic.  

Dr. Crouch has no financial interest in Hamilton City Drug or the compounded medications or metabolism pill formulations supplied by Hamilton City Drug. Dr. Crouch's only interest is to recommend preparations that will promote success, safety and well-being of his patients.  

Notice of health information privacy practices



At Crouch Clinic, P.A., we are committed to treating and using protected health information about you responsibly. This Notice of Health Information Practices describes the personal information we collect, and how and when we use or disclose that information. It also describes your rights as they relate to your protected health information. This Notice is effective April 14, 2003 and applies to all protected health information as defined by federal regulations.

Understanding Your Health Record/Information

Each time you visit Crouch Clinic, P.A., a record of your visit is made. Typically, this record contains your symptoms, examination and test results, diagnoses, treatment, and a plan for future care or treatment. This information, often referred to as your health or medical record, serves as a:

  • Basis for planning your care and treatment,
  • Means of communication among other health professionals who may contribute to your care,
  • Legal document describing the care you received,
  • Means by which you or a third-party payer can verify that services billed were actually provided, and
  • Tool with which we can assess and continually work to improve the care we render and the outcomes we achieve.

Understanding what is in your record and how your health information is used helps you to: ensure its accuracy, better understand who, what, when, where, and why others may access your health information, and make more informed decisions when authorizing disclosure to others.

Your Health Information Rights

Although your health record is the physical property of Crouch Clinic, P.A., the information belongs to you. You have the right to:

  • Obtain a paper copy of this notice of information practices upon request,
  • Inspect and copy your health record as provided in 45 CFR 164.524,
  • Amend your health record as provided in 45 CFR 164l528,
  • Obtain an accounting of disclosures of your health information as provided in 45 CFR 164.528,
  • Request communications of your health information by alternative means of at alternative locations,
  • Request a restriction on certain uses and disclosures of your information as provided by 45 CFR 164.522, and
  • Revoke your authorization to use or disclose health information except to the extent that action has already been taken.


Our Responsibilities

Crouch Clinic, P.A. is required to:

  • Maintain the privacy of your health information,
  • Provide you with this notice as to our legal duties and privacy practices with respect to information we collect and maintain about you,
  • Abide by the terms of this notice,
  • Notify you if we are unable to agree to a requested restriction, and
  • Accommodate reasonable requests you may have to communicate health information by alternative means or at alternative locations.

We reserve the right to change our practices and to make the new provisions effective for all protected health information we maintain.

We will not use or disclose your health information without your authorization, except as described in this notice. We will also discontinue to use or disclose your health information after we have received a written revocation of the authorization according to the procedures included in the authorization.

Examples of Disclosures for Treatment, Payment and Health Operations

We will use your health information for treatment.

For example: Information obtained by a nurse, physician, or other member of your health care team will be recorded in your record and used to determine the course of treatment that should work best for you.

We may, in some instances, use your health information for payment.

For example: A bill may be sent to you or a third-party payer. The information on or accompanying the bill may include information that identifies you, as well as your diagnosis, procedure, and supplies used.

We will use your health information for regular health operations.

For example: Members of our staff may use information in your health record to assess the care and outcomes in your case. This information will then be used in an effort to continually improve the quality and effectiveness of the healthcare and service we provide.

Business associates: There are some services provided in our organization through contacts with business associates. Examples include physician services in the emergency department, radiology, and certain laboratory tests. We may disclose your health information to our business associate so that they can perform the job we’ve asked them to do. We require the business associate to appropriately safeguard your information.

Communication with family: Health professionals, using their best judgement, may disclose to a family member, other relative, close personal friend, or any other person you identify, health information relevant to that person’s involvement in your care or payment related to your care.

Public health: We may disclose your health information to public health or legal authorities as required by law.

Law enforcement: We may disclose health information for law enforcement purposes as required by law or in response to a valid subpoena.

Federal law makes provision for your health information to be released to an appropriate health oversight agency, public health authority or attorney, provided that a work force member or business associate believes in good faith that we have engaged in unlawful conduct or have otherwise violated professional or clinical standards and are potentially endangering one or more patients, workers, or the public.

We are located one block south of the court house square and just behind the Post Office in Hamilton, Texas. Hamilton is located in Central Texas at the intersection of Highway 281 and Highway 36.
